Coaching Evaluations

The Sault Major Hockey Association has developed a  questionnaire with the purpose of evaluating our coaches within the organization. We ask that all parents take the opportunity to provide coaching evaluation feedback. 

Please be honest and objective when submitting your response. Your feedback assists in the selection process of next season's coaches and also helps SMHA to contribute to coaches development by identifying successes and opportunities for improvement.

We value your input and hope you will take the time to complete this form.  Every evaluation will be viewed and forwarded. 

The feedback on this form is considered confidential and family names will not be shared unless you indicate on the form that you wish to be contacted.
